Dear UHA Patient,
As you are aware, our practice is no longer a participating provider of UHA. Despite their actions, we would still love to continue caring for you and your families in the office. You can easily do so even if we are out of network. You do not have to choose another physician as they are suggesting. You can come to the office as usual. However, patients that are established at our practice will need to pay $150 up front for their visit. Our billing department will submit the claims on your behalf. You should expect a check for reimbursement within 30-60 days for your visit.
